Creative Care Consultants assist the older adult and their family through the self-determination process by gathering complete medical, social, and environmental information and presenting available options and choices. This allows us to provide comprehensive care management that fits the needs of elders and their families

Assessment

By evaluating the older person in their present environment, we can best determine their functional abilities, medical condition, mental status, family involvement and financial resources. We utilize a social services/nurse team approach to assure that all their physical, mental, emotional and environmental needs are addressed. At Creative Care Consultants we make the wishes, values and preferred environment of the elderly adult part of our recommendation. We evaluate the ability to live alone and provide resources and/or referrals that enable the older adult to stay at home as long as possible.

Evaluation is conducted in the elder’s present environment and includes:

  • Safety

Care Planning

After assessing the need for short- and long-term care, in-home assistance, the most appropriate living situation, the financial resources available and the level of responsibility the family can take, we provide you with a variety of options, along with a cost analysis of each one. We inform you of all available resources, facilities and programs and help you develop a care plan that best fits your needs, while including the older adult in the decision-making process as much as possible.

After we have worked with you to develop the best possible care plan to meet the specific needs of the older adult, we are there to assure that the care plan is put into place. Creative Care Consultants continue to monitor the individual’s health and well-being, and provide mediation when needed. We can assist with routine services, including transportation needs and coordination of medical care. Many out-of-town families utilize our services on an on-going basis. We are able to take on some of the responsibilities that they would have if they were available, such as escorting the older person to medical appointments and monitoring their care. We keep families informed of their loved ones’ status through written monthly care management updates.

Crisis Intervention

Creative Care Consultants always has a staff member on call if one of our clients has an emergency. If a crisis arises, we are available to react on an immediate basis to reassess the situation, (in or out of the hospital), communicate with physicians, arrange for post-hospital care providers, and identify the need for short- and long-term care. Our expertise in working with older adults enables us to meet challenges as they arise and make necessary recommendations based on the individual’s needs.
